About This
Survey
This study is being conducted by Dr Simon
Denny from the Centre of Youth Health at Kidzfirst Community Health.
The study is being supported by Otahuhu College and Southseas
Healthcare.
The purpose of this study is to find out about
the quality of health care students get and if students have problems
getting health care. To do this we need to ask you questions about
your health and what sort of clinics or doctors they you go to
for your health.
Privacy
The responses to this survey are kept private and confidential.
In any sort of report we might present or publish, we will not
include any information that will make it possible to identify
you. You are not required to do this study, and you can withdraw
from the study at any time. Your decision to take part will not
affect your current or future relations with the Health Clinic
